AUTHORIZATION FOR COLLECTION OF PERSONAL INFORMATION
I authorize the East Lambton Minor Hockey Association (ELMHA) to collect personal information appropriate to the Minor Hockey position applied for by verifying the character references I have supplied.
I also understand that if I am selected for a coaching position I must provide additional personal information in order that ELMHA may conduct a criminal record check.
I understand that the information obtained from the character references and criminal record check will be held in confidence.
