In response to the Ministry of Health releasing the Proof of Vaccination Guidance for Businesses and Organizations under the Reopening Ontario Act on September 14, 2021, OMHA held a meeting this past weekend to review the OMHA Vaccination Policy. Due to the uncertainty of the situation since the September 14 government announcement, ELMHA has decided to extend the date for PLAYERS, COACHES, OFFICIALS, VOLUNTEERS or STAFF, 12 years of age or older, to submit proof of first or full vaccination or an approved exemption letter, until Wednesday, September 22nd.
The ELMHA board requires this information for season planning, especially in determining team sizes. The ELMHA board and coaches require this information to determine team structures and player placements. Therefore, players that do not provide proof of a first or full vaccination or an approved exemption letter by Wednesday, September 22 will be considered ineligible for the 2021-2022 season for our planning purposes.

Any eligible ELMHA participant unable to show proof of first or full vaccination or exemption letter by September 22 will be deemed ineligible to participate after October 31, 2021. Please contact an ELMHA board member if you require support to upload this documentation.
