Mandatory Concussion Baseline Testing
There will be a
mandatory concussion baseline test again this year for all players PEEWEE to
MIDGET on August 25th from 2-6pm and the 26th from 9am-1pm at the Watford
arena.
Parents are asked
to sign their children up for a time slot by following the link below:

For this we ask that families registering more than one player to please add each name separately in the signup genius so that everyone gets their own access code.
Please note, once you have signed up using the signup genius you will receive an email from Dr. Hurley that contains an individual access code. You can pre-register as well as complete an online portion of your test with this code.
New this year, players are required to do an online patient registration before showing up to their appointment, as well as the computer portion of the test. If you do not have computer access you can go directly to Dr. Hurley's office.

Also new this year, any player over the age of 8 years old can be tested using the new system. However, it will be at the player’s additional expense. Anyone interested can contact me for further information as well as pricing.
Any player that is unavailable to make this clinic will be required to make an appointment with Dr. Hurley's office and get tested before they will be allowed on the ice for tryouts - no exceptions. It may take some time to receive the access code from Dr. Hurley, so please try to signup ASAP.
