Mandatory Concussion Baseline Testing- Forms Required
Please review information regarding the baseline test and complete the consent form required for the testing.
Completed forms can be either emailed to info@middlesexspineandsportclinic, faxed to
519-913-5416 or bring it with you the day of the test.

Please arrive to appointment 15 minutes prior to your scheduled time. Please check schedule for times as some times have changed slightly.

We help the youth of our community in many ways as possible (team sports, youth dances, literacy programs, HOBY, VIP programs and more).
